Important Information

WE PARTICIPATE with many insurance plans, including:

Aetna, Anthem Blue Cross, Care Partners Medicare Advantage, Cigna/GreatWest, Coventry/First Health, Connecticare, Medicare, Multiplan & PHCS, Harvard Pilgrim, UHC/ Oxford, Railroad Medicare, Tricare East (Humana Military)

Sorry, we do not participate with Medicaid.

If your plan requires a Referral to be seen by a Specialist, please obtain it from your Primary Doctor before your visit.

Our online scheduler allows patients to self-schedule up to 90 days into the future. If you are an established patient, you should see a “welcome back!” notice appear after validating your name and date of birth. If you do not see this even though you have been a patient of the practice, please call the office after scheduling your appointment to verify your information, before filling out the registration form which will be e-mailed to you.

For Surgical or Cosmetic appointments, you will need to contact our office to schedule.

Registration

After scheduling an appointment, you will receive a registration link from eDerm.

Login with the patient’s last name and date of birth. If you didn’t receive the email, please check your Spam Folder. If you still can’t find the email or can’t login, please call the office.

 

New Patients:

New Patient Forms are found on our website home page under patient forms. We ask that you print and complete these forms and bring with you at the time of your visit.

 

If you are a new patient, we ask that you arrive 15 minutes ahead of your appointment.

 

Returning Patients:

For continuity of care, we recommend you select your established provider for online appointments.
